Who do I contact to cancel an annuity application, the agent or the insurance company, or both?

Changed my mind 3 days after application, told the agent & put stop payment on check mailed from Fidelity transferring funds, but NYLife will still receive the physical check. Agent said it’s all good, nothing has been opened, but do I need to contact NYLife directly as well?

You should contact the insurance company directly.
